Our Programs
Coordinated Life Services (CLS)
Coordinated Life Services provide intensive, personalized life planning and coordination for individuals at risk of entering--or transitioning out of--restrictive settings such as developmental centers.
The program’s goals are to:
- Provide a reliable safety net
- Improve access to essential support services
- Help individuals remain in their communities
- Enhance overall quality of life
Independent Living Services (ILS)
Empowering Individuals to Thrive at Home and in the Community

Through personalized support delivered at home and in local communities, ILS helps individuals build skills for greater independence. This service is ideal for those who need minimal to moderate support.
Coordinated Family Services (CFS)
Guiding Families Through Complex Care Journeys
CFS offers personalized planning, advocacy, and resource navigation for families caring for a loved one with developmental disabilities. The program helps families maintain stability and well-being while building sustainable care plans tailored to their specific circumstances.
